Starting therapy can feel like a big step. You don’t have to figure it out on your own. We’re here to help guide & support you. Every step of the way.
Step 1: Complete the referral form for your chosen program.
This will give us your basic information, tell us what you are seeking support for & help us get you connected with the right provider
(Psychiatric medication management is only available for clients active in one of the below programs)

Step 2: Registration Forms
This will give us all of your information and consents needed to begin services. This needs to be completed before you can see a provider.
Please note: Teens 14 and older are viewed as "adults" in mental health and must also sign their registration forms.
Step 3: Scheduling
Once your paperwork is submitted, we will pair you with the best matched provider. Our office will contact you to schedule an initial intake with your provider.
Step 4: Release of Information (Optional)
If there is anyone your provider may need to talk to, or get records from, please complete this form to allow us to do that.
Please note: Teens 14 and older are viewed as "adults" in mental health. A signed release from the teen is required for any contact by our agency with the parent/guardian

We know that people want to see a therapist quickly. We try to connect new clients with a therapist within the same week of their referral. The first appointment is typically scheduled within 1-3 weeks, based on your schedule and the therapist's availability.

We accept all major private insurances, as well as Medicaid (BadgerCare) and Medicare. We also work with some EAP providers and offer private pay options as well.
It is recommended to check with your insurance company to make sure we are in-network.

Our prescribers are available for clients actively engaged in ongoing therapy at our clinic. To access our Psychiatric Clinic, clients must consult with their therapist for a referral.

No. We offer in-person, video and phone sessions for outpatient services.
