Referral Process
1. Complete the referral form for the program you are interested in.
2. After the referral form is completed, one of our clinical team members will review and reach out to schedule an intake meeting with the parent/ guardian.
3.Once an intake is completed, our treatment team will schedule an assessment at our clinic with the child. These assessment usually last between 1-2 hours.
4. Once assessments are completed, the treatment team will create an individualized treatment plan to submit for approval to the payer (usually insurance or state of Wisconsin)
5. Once approved, we will contact the parent/guardian to select a start date and develop a consistent weekly schedule for services.
6. Services begin!

Social Skills Groups
The Social Skills group offers a large array of social skills that are practiced in a collaborative / inclusive approach. These skills are taught in a structure setting with peers to offer more opportunities to practice the skills learned. Various Levels and age groups available.

CLTS stands for Children’s Long-Term Support. It is a Wisconsin Medicaid waiver program that helps children with disabilities (including Autism and other developmental, physical, or emotional needs) access supports and services in their home and community.

Daily Living Skills: Support for self-care (eating, dressing, hygiene), assistance with safety awareness and independence.
Family Support and Training: Guidance for families and caregivers to manage behaviors at home and in the community, parent coaching to encourage progress outside of therapy.
Social Skills Groups: Our social skills programs help children develop essential social skills in a fun, collaborative, and supportive setting. We focus on promoting inclusion and creating positive peer experiences that extend beyond therapy—supporting children in school, at home, and in the community. Through guided activities, peer collaboration, and recreational play, children learn how to:
Build and maintain friendships
Practice teamwork and cooperation
Strengthen problem-solving and conflict resolution skills
Manage emotions in healthy ways

The length of treatment varies depending on your child’s individual goals and progress. Some families participate in short-term, focused therapy, while others benefit from longer-term, comprehensive support.

We provide center-based services at our clinic, with structured availability. Depending on your child’s needs and funding source, we may also support home- or community-based services.

Social skills groups bring children together in a supportive and collaborative environment to practice communication, friendship management, conflict management, emotional regulation, sharing, problem-solving, and teamwork—all through play, structured activities, and guided practice.
